I here by apply for reservation for my child at YMCA Camp Classen. I agree to pay a $100 non-refundable, non-transferable deposit for each session chosen with this registration to guarantee a spot for Summer Camp 2012. Iunderstand that one session change is allowed without an extra fee but,$50/change will be added after the first change. Registered campers who do not cancel their session at least 14 days in advance, or do not arrive on opening day, may be ineligible for a refund of session fees. If he/she is accepted for enrollment. I understand the terms covering payment of camp dues, and here by give my approval and consent to the application.
INCASE OF MEDICAL EMERGENCY, I hereby also give my permission to the physicianselected by the Camp Director to hospitalize, secure proper treatment for,and to order injection, anesthesia or surgery for my child. I accept theresponsibility for medical and surgical treatment charges which may be incurredon my child’s behalf.
I AUTHORIZE the YMCA to use photographs, slides, video tapes, or digital images of the person named on this registration for its records, public relations or internet programs
