Main Camp Registration

We want to do all we can to make camp available to all families. Realiziing families have different abilities to pay, we are instituting a voluntary three-tier fee program. You may choose the tier that is most suitable to your family. No Matter which tier you choose the experience is the same.

Tier I is out historically subsidized rate and does not reflect the true cost of operating summer camp programs.
Tier II is our partially subsidized fee for those who can pay a little more but still cannot afford the true cost of camp.
Tier III more closely accounts for the true cost of camp including wear and tear of buildings and grounds.

You may request of to 2 cabin mate preferences of like age / gender.  Please keep in mind that an awesome part of camp is making new friends
Below you will be asked to identify someone as parent / guardian 1 and someone as parent guardian 2.  Parent / Guardian 1 should be camp's primary contact for billing and enrollment questions.  Parent / Guardian 2 is in place of a secondary contact.

Permission to attend and Parent Authorization - Please read Carefully
I hereby apply for reservation from my child at YMCA Camp Classen.  I agree to pay a  
$100 nonrefundable, nontransferable deposit is required to guarantee a spot for Summer Camp 2010. I understand that one session change is allowed without an extra fee but, $50/change will be added after the first change. If he/she is accepted for enrollment, my credit card will be charged for the non-refundable, non-transferable deposit.  I agree to pay total camp dues BEFORE OPENING DAY OF THE SESSION(S). I understand the terms covering payment, and give my approval and consent to the application.

IN CASE of MEDICAL  EMERGENCY, I hereby also give my permission to the physician selected by the camp director to hospitalize, secure proper treatment for, and to order injection, anesthesia, or surgery for my child.  I accept the responsibility for medical and surgical treatment charges with may be incurred on my child's behalf.

I Authorize the YMCA to use photographs, slides, and / or videos of the person named on this application for its records, public relations, or internet programs.

Submission of this form constitutes acceptance of the above Parent's Authorization.
